All pets left for boarding must be current on all required vaccinations and free of internal parasites (have a current fecal exam and ogs must have a negative Heartworm test) and external parasites, or they will be treated on admission at the owner's expense.

If my pet becomes ill, a doctor may examine and treat as he/she deems necessary. I give my permission for Murphy Road Animal Hospital, P.C. to administer such treatment and/or medication and I agree that I am responsible for all charges, including an examination charge.

Murphy Road Animal Hospital, P.C. will take reasonable precautions against injury, escape, or death of your pet. The hospital and/or staff will not be held liable for problems that develop provided reasonable care and precautions are followed.

I further realize that I am responsible for payment for the above procedures and treatments in full at the time of discharge. If I neglect to pick up this animal within five days of written notice, you may assume that the pet is abandoned. Abandonment does not release me from my obligation for the bill. I will be responsible for finance charges of 1.75%/month (21%/year) and any collections and/or attorney fees.
